New patients are welcome to join the Practice as long as they live in the Practice area. Anyone who wishes to register with the surgery should call 01923 845 945 and make an appointment with Ms. Jo Williams, Registration Administrator.

Please bring your medical card with you and proof of address and your Passport or Birth Certificate. Before you will be registration is completed all new patients must have a new patient  health check involving an appointment with a Nurse. The patient will have their urine, blood pressure and weight checked, asked some simple questions about their past and current health condition to be added to medical record.

Pre-registration Form

When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records from your previous surgery.   When you come to the surgery you will be asked to sign a form to confirm that the details are correct.
