Registration

New patients are welcome to join the Practice as long as they live in the Practice area. Anyone who wishes to register with the surgery should collect the relevant forms from our reception and return once fully completed with all relevant information/documentation required.  Failing to do so will result in us not being able to process your registration efficiently.

Please bring with you proof of address and your Passport or Birth Certificate. 

When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records from your previous surgery.   

The registration process can take up to 10 working days. Please take this into consideration if you wish to make an appointment with a doctor or the nurse.
